After more than 30 years as Community Health Partnership (CHP), we’ve stepped into a new chapter with a new name.
Our evolution to ChangeLine is the result of careful reflection—about who we are, the work we do, and what it will take to meet the future with clarity and purpose. It captures the role we’ve grown into and the impact we’re here to make.
Founded in 1992 by local healthcare leaders, CHP began as a collaborative focused on improving coordination between providers.
But as the needs of our community grew, so did our mission. We assumed a broader role, serving as a convener and facilitator, bringing people and organizations together to to address systemic issues beyond healthcare.
From Conversations to
Collective Action
Over time, we moved from simply convening conversations to helping strengthen relationships, align strategies, and co-create solutions aimed the root causes of community health challenges.
This evolution led us to a fundamental truth: collaboration alone isn’t enough to solve the challenges our communities face. Lasting change requires more than just bringing people to the table. It demands new ways of leading, different ways of thinking, and a willingness to shake up the status quo.
With that shift came the need for a new identity – one that reflects not only who we are but what we do.
ChangeLine speaks to what we are today: a civic hub that brings people, ideas, and organizations together to take on challenges too big for any one group to solve alone.
But we don’t stop there.
We act as a conduit—actively channeling collective power into coordinated action, helping communities move from good intentions to meaningful impact.
The Next Chapter
A lot has changed over the last 30 years, and our new name reflects that.
But the heart of what we do remains the same: building the collective power of people to take on hard problems and build something better.
We wouldn’t be here without those who have helped us get this far and we know we can’t move forward alone.
