Annual Report: This is what change looked like last year
Our 2025 annual report is here—and it tells the story of a year shaped by change, both for us as an organization and for the community we’re part of.
Internally, we stepped into a new name and identity. After more than 30 years as Community Health Partnership, we became ChangeLine—a shift that reflects how our role has evolved. As a civic hub, our work is rooted in connection: bringing people together, strengthening relationships, and supporting the kind of collaboration that leads to lasting solutions.
But change isn’t just something you name. It’s something you build with the help of others.
Here’s a glimpse of how our work took shape last year:
Community-led healing and leadership: In its first full year, Prism Community Collective welcomed nearly 600 unique visitors—growing into a space shaped by and for the community it serves.
A stronger homelessness response: A record number of volunteers helped lead a more inclusive Point-in-Time Count, creating more accurate data and a stronger foundation for action.
Stronger rural behavioral health systems: In Teller County, deeper coordination and locally driven efforts are improving how communities respond to mental health needs.
Expanded access to care: Through provider training and network-building, more LGBTQ+ people in our region are able to find inclusive, affirming care.
A coordinated approach to suicide prevention: More than 70 organizations aligned around a shared commitment to saving lives—showing what’s possible when efforts are connected.
This is what change looks like but none of it happened on its own. It’s the result of people all across the PIkes Peak region showing up—in conversation, in collaboration, and in action—to navigate complexity and build something better together.
